  1. Do I need to show up early for my appointment?
    a. YES. Please arrive 10-15 minutes before your scheduled appointment time. If you are a new patient, please arrive 25-30 minutes prior to your appointment time to complete New Patient forms.
  2. How long will it take me to get an appointment?
    a. Same day appointments are available in most cases. If you call late in the day, you may have to come for the “walk-in” clinic the next morning.
  3. What should I bring to my appointment?
    a. Please bring your insurance card, a photo I.D., a list of all current medications, and a list of questions/concerns for our providers. When possible, please complete Patient Packet Forms prior to your appointment and bring them with you; these forms are also available on-site.
  4. What insurance plans do you accept?
    a. Our facility is always expanding our insurance base in order to be the most inclusive primary care facility in the valley. Please call our office staff so we can inform you on an individual basis.
  5. If I am coming in for a Department of Transportation Physical do I need to bring anything with me?
    a. Although you will fill out different paperwork, we will need an accurate list of medications along with any supporting and relevant medical documents such as recent labs if diabetic or CPAP/BIPAP compliance report if using a machine for sleep apnea. Please come prepared with medical history reports if appropriate. Please call our office with any individual questions about a D.O.T. Physical.
  6. I am interested in aesthetic procedures; can I come in for a consult scheduling a procedure?
    a. Yes! We offer free consultation for our aesthetic patients in order to properly assess them. Please keep in mind cosmetic procedure are not usually covered by insurance.
  7. What do I do if I have an emergency after hours?
    a. If you are have a medical emergency please call 911 or go to your nearest emergency room or urgent care. If you need a medication refill or appointment, please call our office and leave a message and a member of our staff will get back to you within the next business day.
  8. Can you treat my whole family or do I need a pediatrician?
    a. We treat patients as young as newborns and up to any age.
  9. What is an “NP” or Nurse Practitioner?
    a. NP stands for Nurse Practitioner. A Nurse Practitioner must first obtain a Bachelor’s Degree in Nursing, then attend postgraduate training and graduate with a Master’s Degree in Nursing. NP’s also are Board Certified and take continuing medical education courses yearly. They treat patients as young as newborns and are excellent providers for “sick” visits. They can also perform Women’s Well Exams, PAP-smears, and Sports Physicals. In addition, NPs perform Adult Well Exams in our office and also treat patients for Depression/Anxiety.
  10. Do I need to send my old records to you prior to my first visit?
    a. Old medical records are useful to have for patients with complicated health histories, but for the most part, old records are not necessary for the first visit.
  11. Can I be assured of total confidentiality?
    a. Each of our employees has signed a confidentiality agreement regarding the use and care of your medical information. This is a cardinal rule for our practice. We understand our legal obligations to protect your privacy according to the privacy provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), which is federal law. Ask us at any time about any of your concerns regarding privacy and confidentiality.
